As an NDIS service provider, we strictly adhere to the guidelines set by the NDIS Quality and Safeguards Commission for incident management and reporting. This policy outlines our practices for accident prevention, incident management, and compliance with NDIS standards.

Accident and Incident Prevention

Family Instructions:

Families are provided with safety guidelines, including information on accessing permitted areas and proper use of equipment.

  • The death of a client under our care.

  • Serious injuries occurring during or connected to our services.

  • Physical, emotional, or sexual abuse, neglect, or financial exploitation.

  • Unauthorised restrictive practices.

  • Immediate reporting: Incidents must be reported to the supervising therapist as soon as they occur.

  • Documentation: Staff will complete a detailed incident report form within 24 hours.

  • NDIS notification: If the incident qualifies as reportable under NDIS guidelines, it will be reported within 24 hours via the NDIS Commission portal. A detailed follow-up report, including corrective actions, will be submitted within five days.

  • Family notification: Families will be informed of any incidents involving the client and the measures taken to ensure safety.

  • Evaluation: A review will be conducted to identify the cause of the incident and prevent future occurrences.

All incident-related data will be handled confidentially, respecting the privacy of the client and their family.

Incident Management

Definition of an Incident:

An incident is any unexpected event that may compromise the safety, health, or well-being of a client, caregiver, or staff member. This includes serious injuries, abuse, neglecr or explotation and/or situations that pose a risk to safety.

NDIS Requirements for Reportable Incidents:

In line with the NDIS Quality and Safeguards Commission, Neuronaut will promptly report the following:

Post-Incident Follow-Up:

Meetings will be offered to families to discuss the incident, the client’s progress, and any necessary adjustments to the therapy plan.

Accompaniment Requirement:

If a client attends with more than one child, it is mandatory to have an additional adult present. This ensures:

  • One adult accompanies one child during the therapy session.

  • The other adult supervises the other child in the waiting area.

Supervision:

Neuronaut does not provide childcare services in the waiting area. Families are responsible for ensuring that children are supervised by the additional accompanying adult.

Exceptions:

If the therapist planned a therapy session with all the children together at the same time (for example, to see how they interact with each other), one accompanying adult is enough.

Non-Compliance:

If only one adult arrives with more than one child, and the therapist did not pre-schedule a group session, the session will be cancelled for safety reasons. This will be subject to our cancellation policy, and a fee equivalent to 100% of the session cost will apply.

This policy will be reviewed annually to ensure compliance with current NDIS regulations and best practices in safety and incident management.

Mobility Support:

  • Accessible infrastructure:

    • Non-slip ramps, wide access points, and obstacle-free areas.

    • Therapy and waiting spaces designed to accommodate mobility devices.

  • Physical assistance:

    • Staff are available to assist with movement within the facility.

  • Emergency protocols:

    • During evacuations, priority will be given to safely transferring clients with mobility challenges.

Sensory Needs Support:

  • Sensory-Sensitive environments:

    • Low-stimulation areas for clients sensitive to noise, bright lights, or crowds.

    • Access to sensory tools, such as noise-cancelling headphones and sensory toys.

  • Trained staff:

    • Strategies for de-escalation and neuroaffirmative support to prevent sensory overload.

Personalised safety plans:

Each client undergoes an individual assessment to identify specific risks and establish a tailored safety plan.
